1. Be sure to:
1. Completely fill out the application in the manner requested.
2. Submit (2) personal references + (1) pastor reference
3. Enclose two photographs of yourself (passport size photos) for our records and your student ID card.
4. Enclose a copy of your high school diploma or GED certificate, and your high school or college transcripts (if you do not have access to these copies currently, you can submit your application now and forward copies of your transcripts at a later time)
5. Sign and date the application. 
6. Mail the application with a $25.00 application fee to:
